My driver’s side seat track, left side lost one of the rollers so the seat rocks forward. With the extended cab, the track has a release set up so that a lever on the seat back will simultaneously release both the seat back to allow it to fold forward and the seat bottom slides forward with spring assistance on the tracks to allow access to the back seat with out having to go double steps.
The one on the seat bottom is for reclining angle only.

I’ve not been able to find a replacement in any of the yards around my place in Utah and BigT has kept an eye out too. As an interim I got one out of a regular cab and just made due.
Last Friday, BigT was in Victorville making rounds to find parts for his myriad of projects and located a one that might work but, bench seat.

Knowing I was headed there this weekend for the Land Speed races at El Mirage, I didn’t ask him to tear the seat out just to see if it was comparable. It looked close but, odds are against. That and the fact that it was hot as, well, it was hot and he’d been out there hours…friends don’t abuse friends like that.

So, yesterday, I swung out there and SCORE. Perfect match to my old one.

And, as a bonus, full cup holder assy since the flip out holder busted.

Some interior color match paint after some clean up and the seat and cup holder will be back to normal again.

I have a leather version of yours in my '95 but mine has never slid forward even though it shows that is should based on the picture on the lever arm. Passenger side does just fine, though. I remember my seat frame was broken when I got the truck and needed welded. I suppose I should take it back out to see what I can find or maybe take both out to compare the mechanisms.

Glad you got yours working well again.

I'm willing to bet you've got what I used for a work around until I found this one. Weird how there are so many versions, even of the same thing. I pulled the slide out cup holder from the 95 in the yard and even it is slightly different from the one I'm replacing in my 96.
