The source I used for my replacement heater is out of stock as well. The one you listed from Amazon looks correct to me too. Fair enough price since it is hard to find anymore.
 
I am not sure if replacing the whole FFM is possible?
I know you have put a lot of mod in your truck.
Or buy the whole FFM to harvest the heater? to preserve the FTB mod.
At least, hopefully, you get the correct heater.
 
Problem solved, here's what I bought for $127 on Amazon: ACDelco 10226035 Professional Fuel Filter. With it I get the heater, water in fuel sensor, new O-rings, and a filter. I'm not using the can since I would have to modify it for FTB. The heater in this thing looks nothing like the ones in my OP but now I know this will work.
 
This is the old one, the new one is already installed but looked the same. There are no numbers on the part. It's nothing like the photos on Leroy's or Amazon, those might be some stock photos of a heater but not this one. They might still work I just didn't want to take a chance and end up doing what I did anyway.
