I know you must remove the steering box because there is not enough clearance to get the pitman arm off. You might be able to just remove the steering box bolts and get enough room to wiggle it off. Of course you need a puller to get the arm loose first. I think that is where the clearance...

Cover the Turbo shield with a bath towel for a few seconds and see if that cancels the noise. If it does, that is what John is talking about. If it is the "chink chink" noise I hear that is what it is. Kinda common with these trucks....
I don't think that the vacuum pump on these was designed for a brake booster. The booster has a large diaphragm. I think the pump should pull about 20". I would have tested it on the engine though.
I had 2 PMD failures 2 years apart. Both of them happened at the same time of day in the same place when I was leaving work. First failure was stock location and the second was SS Diesel location (intake).
