Typical life of injectors is 100K so you are due for a fresh set, IP seal fail is not a common fail mode could happen I guess but not common, I'm inclined to suspect injectors,

Could be worn injector/s filling up the cyl and washing into CC, how many miles on the current set, also any smoke out the tail pipe while idling if so what color ?

test for lift pump output is pretty basic and requires vehicle to be driven to know for sure as test at idle only tells part of the story, does not let you know of partially blocked sock or if lift pump is getting reduced output, load test as outlined in the TRL...
Pinhole in return would cause a fuel leak externally, but would have no bearing on operation beyond that; the return is pressurized albiet very low pressure so when engine is running no way for it to suck air and that then causing operability issues, more to your buddies story I think
