Right output shaft falling out

On sons truck we found the right output shaft backed out about 2" for the housing today.
Can't say I've ever seen that one before.
Needless to say the front did won't engage for 4x4.

Tomorrow he will get his first lesson on diff. repairs.):h
 
Found a smashed shim and thrust washer.
Going to replace them along with a new snap ring outer seal.
Also upper and lower ball joint & 1/2 shaft.

At this point I don't know what did it.
I'm not seeing any damage other then the result, not the cause.
The ring inside the spline connector maybe..
Driver error maybe..

I'll look at the ring tomorrow for a flat area on it.
The splines and grove on the shaft and connector look good.
