When I accelerate heavily there is one big puff of black smoke and then it clears up.
Is this normal?

its normal. The turbo has to spool up before it can clean up the smoke

Normal but varies with age/condition of overall health. At 60K miles my truck was pretty smokeless once I upgraded exhaust. Then got worse with age. The puff was reduced with new injectors at 120K. I am sure my IP is getting tired now as well as injectors and the puff has been getting worse and worse. If I dose heavy FPPF 8+ Cetane improver the smoke is lighter. If fuel filter is restricted and IP is low on LP fuel pressure/supply it will smoke worse too.
