NV5600 in a 03 Dodge ticking or exhaust leak?

Looking at a 3500 Dodge diesel that has a ticking in most forward gears. Sounds like an exhaust leak with more throttle the noise is louder. Doesn't do it out of gear with the clutch out or in. Slowing down it doesn't do it. Has an exhaust brake on it. Supposedly has a clutch done at some point. 100K mikes on it.

Is this a common thing to have problems with or a bad expensive sign?
 
I know the NV5600 was thought to have a lubing issue. standardtransmission out of texas seem to advertise they have a fix. this is more a rumble in the higher gears under load.

as far as a tick though, I never heard of it.
