Energy Suspension Poly Trans Mount

So I got this because I can't really tell if my Trans Mount is ripped, but if I put my foot on the Tail Shaft, I can wiggle the Trans a good amount Left or Right. Which this could also be causing my Wiggling if the Mount is in fact Ripped.

So...of course there are no Instructions with this, and it fits several different Trans / Vehicle Combos, so my Question is...Do I use the extra Plate that it came with for the Install or no? Has anyone else Installed one of these on their Trucks?

Pretty sure the 2nd plate is to stiffen the rubber between the two plates

Can you post the part number for this mount and take some pictures of it installed? Do the height dimensions match with the OEM mount?
